TERMS: I here by request that my child(ren) named above be admitted to the A-Game Sport Camps. In the event I cannot be reached, I authorize the Director/Coach for me according to his/her best judgment in any case requiring attention. I understand that videos and/or photographs of camper may be used for promotional purposes. The camp must comply with the Massachusetts Department of Public Health and be licensed by the local board of health. Parents my request copies of health care and discipline policies and procedures for filing grievances. Required forms due no later than the first day of camp, Waiver, Health Form, Policy, may be downloaded at: www.agamesportscamps.com/forms.htm. Campers must be checked in and out daily at the Governor’s Academy field house. Extended care is $15/day Monday – Friday; 3:00pm – 5:00pm. I agree to the above terms:
